Job Summary: The Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in the home or hospice setting. Care rendered is in accordance with the physician’s orders and under the supervision of the RN Case Manager and Regional Manager.
1. Education: Graduate of an accredited school of nursing. 2. Experience: Minimum of one year in a clinical setting, med-surg preferred. 3. Certification/License/Registration: Valid Licensed Practical Nurse (LPN) in the State of Georgia. Valid driver’s license. Auto liability insurance.
